The purpose of this survey is to help adults in your school better understand what is working or not working about your experience preparing for life after high school.

This survey should take about 10 minutes to complete. Please answer openly and honestly. Your responses will be kept anonymous.

Instructions:

When you read the statements, you should try to think generally about your experiences at your school. If there are any questions you don’t want to answer or don’t know how to answer accurately, feel free to skip them. When you see a reference to “counselors” in your school, it means adults like advisers, college navigators, or other people who you regularly talk to about academics and your plans for after high school. When you see “administrators” in your school, it means principals, assistant principals, and other adults who are in leadership positions.
